Resource Center: Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model

Background


In late 2025, the Centers for Medicare and Medicaid Services (CMS) proposed a groundbreaking new outcomes-based payment program called the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model. ACCESS represents an experiment as to "whether an alternative payment methodology—Outcome-Aligned Payments (OAPs)—for technology-enabled chronic care reduces expenditures while preserving or enhancing quality of care for Medicare beneficiaries." (CMS ACCESS RFA)


Given the innovative features embedded into ACCESS, and the OAPs in particular, InHealth worked with our colleagues at Elevare Law to develop resources to help healthcare organizations prepare for and thrive through participation in ACCESS. This page summarizes these resources.
