Background In late 2025, the Centers for Medicare and Medicaid Services (CMS) proposed a groundbreaking new outcomes-based payment program called the Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model. ACCESS represents an experiment as to "whether an alternative payment methodology—Outcome-Aligned Payments (OAPs)—for technology-enabled chronic care reduces expenditures while preserving or enhancing quality of care for Medicare beneficiaries."
